When you enroll on an IU13 medical plan and are an active employee (not a COBRA/Retiree participant), you have the opportunity to participate in the IU's voluntary Wellness Program. The program design rewards healthy lifestyle choices with the opportunity to reduce your portion of the payroll deductions that you pay for medical coverage! Annual participation in the Wellness Program is voluntary and each year a fresh Personal Health Assessment (PHA) and Health Risk Questionnaire are required.

In order to maximize your benefit with this program, you need to complete the Health Risk Questionnaire in addition to the Personal Health Assessment. Just like past years, the Questionnaire and Assessment work hand-in-hand to get a comprehensive look at your health. You even earn 1 carrot just for completing the Questionnaire. YOUR HEALTH RISK QUESTIONNAIRE: Beginning on February 19, 2024, you will have the opportunity to complete the Health Risk Questionnaire. To get started, you must log in to your HealthPassport account and navigate to the "My Carrots" page - toolbar at the top of the page or from the "Menu" dropdown list in the upper right-hand corner of the screen.
From there, click the red "Click Here to Begin" button to complete the 2024 Health Risk Questionnaire.
Please note that you are not required to answer all questions in the questionnaire. Only those denoted with a red asterisk (*) require a response.
SCHEDULING YOUR APPOINTMENT: Beginning on February 5, 2024, you have the opportunity to schedule an appointment at a "Company Event" or at any CareATC Health and Wellness Center. To schedule your appointment, log into the CareATC Patient Portal (https://portal.careatc.com), select "PHA Scheduling," and then select your preferred PHA site option - "Company Event" or Health Center. The "Company Event" is hosted by CareATC's dedicated PHA Team and appointments are scheduled at 5-minute intervals, allowing a large number of participants the opportunity to screen in a short amount of time! If the "Company Events" do not work in your or your spouse's schedule, the opportunity to complete the PHA in the CareATC Health and Wellness Center at a date or time** that is convenient for you is available.
**Please be advised that the PHA must be scheduled to occur at a CareATC Health and Wellness center between February 19 and April 27, 2024. If a PHA is completed before or after the window, you and/or your spouse will not earn "carrots" based on the results.
YOUR RESULTS: After your PHA, you will receive a text notification to view your results and your personalized PHA Results Video by logging into the CareATC Patient Portal or the CareATC mobile app. The PHA results will also be uploaded by CareATC directly to the HealthPassport site for you to see the total number of carrots earned for the 2024 PHA campaign. If you feel that there is a discrepancy with the results, or you want to request a medical exception for reaching a specific goal, you may file an appeal. All participants have 30 calendar-days from the date that his/her individual results are posted in HealthPassport to file an appeal. YOUR PHA REVIEW: Participants are encouraged to schedule a PHA Review appointment with a CareATC provider to ask questions about individual results and learn how to make improvements for next year's screening.
