"New Patient" Paperwork with CareATC  

While you may not be "new" to the Health & Wellness Centers, you will need to complete some "new patient" paperwork for the new provider. Here are some tips to make that process a little easier and faster.


 
⇒ QUICK - AN IMPORTANT NOTE! As an eligible IU13 employee or covered dependent, you DO NOT need to provide your Social Security Number (SSN) or insurance information. If you are asked to provide this information in person or on any forms, please disregard. (Other CareATC customers typically need to provide this information, but we do not.)
 
Establishing an Account with CareATC:
First, call 800.993.8244 to obtain your temporary username (UN) and password (PW).
 
Once you have your temporary UN and PW, go to the CareATC Patient Portal to establish your account. Log in using the temporary UN and PW, then go to the "Update Your Personal Information" to establish a permanent UN and PW. This must be done within 24 hours of calling the 800#.

Schedule an Appointment with CareATC:
Once you have an account, you can schedule an appointment in three easy ways:
    • Online – Visit careatc.com/patients
    • App – Download the CareATC app and login
    • Phone – Call 800.993.8244
Save Time by Completing the "New Patient" Paperwork in Advance:
While you may not be a new patient to the Health & Wellness Centers, you are new to the CareATC system. Therefore, you must first complete the New Patient Paperwork. To save you some time, you can access the forms in advance of your first appointment by clicking here.

A few notes about the New Patient Paperwork:

  • The forms will ask for your SSN and Insurance Card information. This is NOT required for IU13 participants. Just leave those slots blank.
  • The forms will automatically upload at the end when you click the "finish" button. There is no "submit" button, nor will you receive a confirmation email.
  • There is an option to download and print the forms, if you would prefer this method.
  • This is a requirement for all "new" patients with CareATC. Even if you had your medical files transferred from QuadMed to CareATC, you will be required to complete this paperwork.

  • Make, view, or cancel appointments
  • Refill prescriptions
  • View Personal Health Assessment (PHA) results
  • Access health education
  • View medical records, labs, history
  • View provider bios
  • Location-based check-in
 

*The Health & Wellness Center, Physical Therapy, and Teladoc services are available to staff who meet eligibility criteria and dependents (ages 2 and up) who are enrolled in an IU13 medical plan.
